Advanced Primary Care Initiatives

The Centers for Medicare & Medicaid Services (CMS) is currently seeking input on initiatives for improvement opportunities in advanced primary care. Specific focus areas include:

  • Mechanisms to encourage more comprehensiveness in primary care delivery;
  • Improvements to the care of complex patients;
  • Facilitating robust connections to the medical neighborhood and community-based services;
  • Moving reimbursement from encounter-based towards value-driven, population-based care.

CMS is seeking input from a broad range of groups including healthcare providers, Medicaid agencies, health plans, and several others. All input must be submitted using the Request For Information (RFI) document and received no later than 11:59 p.m. EDT on March 16, 2015.  Additional information can be found here.
