2017 Coding Changes: Spinal Instrumentation

Spinal instrumentation code +22851 has been deleted and replaced with three new codes for specific circumstances:

  • For integral anterior instrumentation for device anchoring to intervertebral disc space interbody fusion;
  • For integral anterior instrumentation for device anchoring to vertebral corpectomy(ies) defect interbody fusion;
  • For intervertebral dis space or vertebral body defect stabilization without interbody fusion

This differentiation of biomechanical device insertion distinguishes the different work, materials, and anatomy.

Report 22853 for each treated intervertebral disc space.

22853, 22854, 22859 may be reported more than once for non-contiguous defects.

Do not use with modifier 62.

In addition, Category III codes 0171T and 0172T were replaced with four new Category I codes: 22867-2870. These codes more specifically differentiate differing types of spinal instrumentation that may be used. At the same level, do not report with:

  • Arthrodesis (posterior, anterior)
  • Laminectomy/Laminotomy
  • Vertebrectomy
  • Diskectomy
  • Imaging guidance for needle/catheter placement for diagnostic or therapeutic injection.
