Mechanochemical ablation (MOCA) includes two modalities delivered to the entire length of the extremity vein(s):
- Mechanical damage to the venous intima, and
- Administration of medication (sclerosant).
36473 – Endovenous ablation therapy of incompetent vein, extremity, inclusive of all imaging guidance and monitoring, percutaneous, mechanochemical; first vein treated
+36474 – subsequent vein(s) treated in a single extremity, each through separate access sites (List separately in addition to code for primary procedure)
These codes are reported per extremity. Code 36474 should not be reported more than once per extremity regardless of number of veins treated or the number of access sites.
Sclerosant injection by either needle or catheter followed by a compression technique is not mechanochemical vein ablation. Report this service with the unlisted code 37799.
These codes may not be reported with radiofrequency or laser ablation for the same vein/same leg/same session.
