One deleted code, 99420, and two new codes, 96160 and 96161.
96160 – Administration of patient-focused health risk assessment instrument (e.g., health hazard appraisal) with scoring and documentation, per standardized instrument
96161 – Administration of caregiver-focused health risk assessment instrument (e.g., depression inventory) for the benefit of the patient, with scoring and documentation, per standardized instrument
This is usually performed by non-physician staff. It is per instrument so multiple units may be reported. We will keep an eye out for a MUE.
The physician/NPP services for interpretation and subsequent planning, etc. is reported separately with an E&M or other face-to-face service.

Codes 96110 and 96127 can be reported separately as long as the instruments are unique. I would anticipate a CCI edit for this
