As of July 1, 2014 the new HCPCS code Q9970 should be used for Injectafer ® (ferric carboxymaltose injection). Code J3490 should no longer be used and will cause a denial for incorrect billing. Injectafer ® (ferric carboxymaltose injection) is an iron replacement product indicated for adult patients for treatment of iron deficiency anemia and who have intolerance to oral iron or have had an unsatisfactory response to oral iron. It also can be prescribed for adult patients who have non–dialysis chronic renal failure. Medical record documentation is important and must contain patient history and physical findings that verify medical necessity for the use of Injectafer.
The drug administration codes are:
- 96374 – Therapeutic, prophylactic, or diagnostic injection (specify substance or drug) Intravenous Push single or initial substance/drug.
- 96365 – Intravenous Infusion for therapy, prophylaxis, or diagnosis, (specify substance or drug); initial up to 1 hour.
