The Centers for Medicare & Medicaid Services (CMS) updated relative value units for CPT code 63650 (percutaneous implantation of neurostimulator electrode array, epidural) to include HCPCS code L8680 (implantable neurostimulator electrode, each). Effective with dates of service on or after January 1, 2015 Anthem will no longer allow separate reimbursement for L8680 when reported with 63650, even when billed with modifiers 25 and 59.
