PCMH Reporting Packages
Two new reporting PCMH packages are available to assist with measures associated with NCQA standards/elements/factors.
PCMH Reporting Package A – a complimentary package of three critical reports required for NCQA PCMH autocredit:

- PCMH 2: Team-Based Care, Elements 2A, 2B, 2C
- PCMH 3: Population Health Management, Elements 3A, 3B
- PCMH 4: Care Management and Support, Elements 4C, 4D, 4E

PCMH White Papers
NextGen Healthcare has completely updated the previous PCMH User Manual by creating a set of complimentary white papers based on NCQA PCMH 2014, the latest available standards from NCQA. Each white paper details the workflow and reporting process for a single PCMH element, including all relevant factors. Many of these are currently posted on the PCMH Resources page Knowledge Exchange. Additional white papers will be posted upon completion of final quality review.to Knowledge Exchange (or the new Success Community once it goes live).
