The new deadline is October 8 for practices to apply for participation in the Centers for Medicare & Medicaid Services (CMS) Million Hearts: Cardiovascular Disease Risk Reduction Model.
Million Hearts is a national initiative to prevent one million heart attacks and strokes by 2017. This initiative brings together both existing efforts and new programs to improve health across communities. CMS is soliciting participation in the Million Hearts program as a way to design and measure effectiveness of a specific model for treatment of cardiovascular disease. The program reimburses providers for participation based on outcomes over a five-year period beginning in 2016.
How Does NextGen Healthcare support Million Hearts?
Participating providers can report Million Hearts measures to CMS individually or through the PQRS Group Reporting Option using NextGen® Health Quality Measures (NextGen HQM), which supports a number of clinical quality measures associated with this program. Clients should also look for information from NextGen Healthcare soon regarding cardiovascular risk scoring as it relates to this program.
