Under the 2015 Physician Fee Schedule providers can bill for chronic care management. This is a single code (99490) for non-face-to-face chronic care management services by the physician or other qualified healthcare professional. Providers meeting the required elements can bill per calendar month and earn $42.60 per month per patient according to the final rule, which was released October 31, 2014.
